Candidates must submit their immunisation record prior to booking in for a medical assessment with 4CRisk. We need to know if you have ever had the following immunisations:

Hepatitis B
Whooping Cough (Diptheria, Tetanus, Pertussis)

Tuberculosis (only required in certain instances)

QAS will accept other forms of official documents and these must include your full name and date of birth
QAS will not accept edited or cropped official documents.

1 - Further Medical Information / Investigations Required from Your Medical Practitioner:

If, during the assessment process, it is determined that you have a medical condition requiring further investigation or additional information from your treating doctor, we will contact you and provide a referral letter for your treating doctor. Once we receive this information from you, our Doctor will review it and make the Fitness for Work Determination.

This fee, totaling $175 plus GST, covers the cost of the referral letter, the review of additional information, and the final fitness for work determination by the Doctor.

2 - Pre & Post Ventolin Spirometry:

In the event that your lung function test results indicate a restriction in your lungs, an additional test may be required to determine if the restriction is reversible using Ventolin. This test is known as a Pre & Post Ventolin Spirometry test and involves a spirometry test followed by the administration of Ventolin and another spirometry test. The cost for this supplementary test is $70 plus GST at our Clinic or $99 plus GST at an affiliated partner clinic.

3 - Cancellation & Rescheduling Your Appointment:

Please be aware that if you cancel or reschedule your appointment with short or no notice, you will be subject to the fees outlined in our Cancellation & Rescheduling Policy, which can be found here:

Next Steps

Confirmation of your Appointment

"You will receive an SMS and email from 4cRisk advising of your appointment details and location. We will also advise you on what to bring to your assessment and other relevant details. It is important that you review this information and understand your requirements prior to attending”.

Attending your Appointment

“You will attend your scheduled appointment, allowing up to 3 hours of time to complete the assessment. Failure to attend your scheduled appointment or cancellation within 48 hours will result in fees being applied”.

Receiving your Medical Results

“Your Medical Results will be assessed by a 4CRisk Doctor as per the QAS Medical Standards. Your Medical Results will be sent to both QAS stakeholders within 48 hours of receiving all required information”.
