4cRisk collects medical and demographic information that is necessary to provide independent medical advice and/or treatment. 

4cRisk is committed to protecting the privacy of individuals and organisations in respect to sensitive information. 4cRisk stores and processes personal and sensitive information in accordance with the Australian Privacy Principles outlined in the Privacy Act 1988 (Cth) (the Act). 

Collection

We may collect the following types of personal information:

  • your name, address and telephone number;
  • your age or date of birth;
  • current drugs or treatments that you use;
  • information relevant to your medical care, including but not limited to your previous and current medical history and your family medical history (where clinically relevant);
  • your ethnic background;
  • your profession, occupation or job title;
  • the name of any health service provider or medical specialist to whom you are referred, including copies of any letters of referrals and copies of any reports back; and
  • any additional information relating to you that you provide to us directly through our representatives, medical or allied health professionals providing services, or administrative staff.

Use of information

4cRisk may use your sensitive information:

  • to provide the results of your medical assessment to the organisation that referred you to 4crisk and/or the owner/operator of the site to which you work or have applied to work. The report may reference information that you or your treating practitioners have provided if relevant for the purpose of this assessment;
  • to facilitate the management of our practice, including account keeping, quality assurance, accreditation, and teaching;
  • to refer you to other medical professionals for treatment or advice, including sending biological samples to pathology for analysis;
  • Where compelled by law.

Data Security

All sensitive information is stored securely and protected by appropriate data security measures and maintained within Australian shores. When no longer required, hard copy documents are disposed of using secure bins.
